T-Mobile Change of Responsibility - Next Steps

Thank you for your interest in transferring your existing T-Mobile line(s) to the Evergreen account! If you have pre-paid service with T-Mobile, please visit here to order a new SIM card.

Steps to Start the T-Mobile Change of Responsibility (CoR) Process


You'll need a “Move Authorization” from your current T-Mobile account to transfer your T-Mobile line(s) to a new account. This is required by the FCC and cannot be skipped.


Here’s what to do:


1. Call 611 from your T-Mobile phone to request a Move Authorization.

2. Once you have the authorization, fill out the fields below and submit your request.

3. Keep in mind:

  • The transfer process takes 7–10 business days.

  • Your Move Authorization is valid for about 14 days.

  • Don’t let a T-Mobile rep complete the transfer for you. Just ask for the Move Authorization.

  • Your balance must be paid in full before you can transfer your line(s).

  • If you have a device installment plan, your equipment must be fully paid off and unlocked
